Longevity has gone mainstream, and few platforms have done more to drive that conversation than The Diary of a CEO, where physicians and researchers regularly discuss aging, energy, cognitive performance, and what it means to stay resilient as we get older. Underneath the headlines, one idea from those conversations stands out — and it happens to be the right one.

Brain Fog Is a Symptom. The Bigger Question Is Why.

When people experience brain fog, low energy, slower recovery, or reduced resilience, the goal should not simply be to mask the symptom. The more important question is: what is happening underneath it?

As we age, the body changes at many levels — immune function, inflammatory signaling, circulating proteins, and cellular regulation all shift over time. Longevity medicine is increasingly focused on understanding these changes and asking whether some of them can be safely influenced.

One area of growing scientific interest is therapeutic plasma exchange, or TPE.

What Is Therapeutic Plasma Exchange?

Therapeutic plasma exchange is a medical procedure that removes a portion of a person’s plasma while returning their blood cells to circulation along with a replacement fluid. TPE is not experimental as a procedure: it has been used in conventional medicine for decades for selected neurologic, autoimmune, and hematologic conditions [1].

What is newer is the question researchers are now asking: could changing the circulating plasma environment influence biological processes associated with aging?

Why Is Plasma Being Studied in Longevity Medicine?

Plasma carries thousands of proteins, antibodies, inflammatory mediators, hormones, and signaling molecules throughout the body — and that circulating environment changes as we age. Researchers are studying whether removing and replacing a portion of plasma may alter some of the circulating factors associated with inflammation, immune regulation, and biological aging [2][3].

Importantly, this is not the same as receiving “young blood.” The research focuses on plasma exchange and plasma dilution — removing and diluting a person’s own aged plasma environment, not transfusing plasma from young donors [3][4].

What Does This Have to Do With Brain Fog?

Brain fog can describe difficulty concentrating, slower thinking, reduced mental clarity, or feeling mentally fatigued. There are many possible causes, which is exactly why brain fog should not be treated as a single disease or assumed to have one solution.

Our approach is to look at the broader medical picture and ask whether inflammation, metabolic health, immune function, recovery, or other physiological factors may be contributing. The goal is not simply to make someone feel temporarily more alert. The goal is to understand why they do not feel like themselves.

What Does the Research Show About Plasma Exchange and Aging?

Early human research has reported changes in several biomarkers associated with biological aging following therapeutic plasma exchange, including a 2025 randomized, placebo-controlled trial that found shifts across multiple epigenetic aging clocks [2] and an earlier clinical study reporting a younger systemic protein profile after plasma dilution [3].

These findings are scientifically interesting, but they should be interpreted carefully. A change in an aging biomarker does not prove that plasma exchange reverses aging, prevents disease, improves longevity, or extends lifespan. Research into therapeutic plasma exchange for longevity remains emerging. Longevity Medicine Should Be Measured, Not Guessed

Longevity medicine should not be based on promises of becoming younger. It should be based on understanding the individual. Depending on the patient, evaluation may include:

  • Brain fog and cognitive symptoms
  • Energy and recovery
  • Metabolic health
  • Cardiovascular risk factors
  • Inflammatory markers
  • Immune health
  • Body composition
  • Relevant laboratory testing
  • Biological-aging biomarkers when appropriate

The goal is not to chase one number. The goal is to understand the systems that may be changing and determine what can meaningfully and safely be addressed.

Therapeutic Plasma Exchange Is Not a Fountain of Youth

There is growing interest in therapeutic plasma exchange and aging biology, but promising research is not the same as proven longevity treatment. Therapeutic plasma exchange has not been proven to reverse aging or extend human lifespan.

Our approach is physician-led and science-focused. We evaluate the available evidence, the patient’s medical history, and the potential risks and potential benefits before determining whether therapeutic plasma exchange is appropriate.

Brain Fog Shouldn’t Automatically Be Dismissed as Aging

Brain fog. Low energy. Poor recovery. Feeling less mentally sharp. Reduced resilience.

These experiences are common, but they should not automatically be dismissed with “I’m just getting older.” Sometimes they deserve a deeper medical evaluation.
